INSTRUCTIONS  

1. BE NEAT! The compact nature of this form requires you to mark accurately in the space provided.

2. Double check all figures for accuracy.

3. Your order will be mailed to the address requested, however, you are urged to provide a campus address.

4. Please retain a copy for your chapter files and check it against your shipment when it arrives.

5. Submit Kappa Kappa Psi and Tau Beta Sigma orders on SEPARATE order forms.

6. If your order has not been received within a four-week period, please notify the National Headquarters.

7. You are responsible for added shipping costs if UPS 2nd Day Air or Next Day Air service is required.
