4. Fall Activity Report Form Due December 1, 2001

All chapters need to be gathering data to submit in their FAR Form which is due to the HQs December 1 (postmarked date). We recommend you not wait until the last minute to track down your Sponsor for signature. Try to get it done before Thanksgiving!

A copy of the FAR Form and cover memo is being sent out tomorrow to all Chapter & District Presidents, and District Governors & Counselors. You may also download a copy from the HQ's website at http://www.kkytbs.org/forms.html.

To help you begin thinking about getting this done, a copy of the memo follows:

FROM: Alan L. Bonner, National Executive Director
RE: Fall Activity Report Form

In order for the Report to be accepted as "on time", it must contain the Chapter President’s and Sponsor’s signatures. This form must be postmarked no later than December 1. If you will be mailing it through a campus post office, you should mail it one day earlier.
